In 1998, he completed his residency at the Cardiovascular Surgery Clinic of the Çukurova University School of Medicine. He has participated in adult peripheral vascular surgery, abdominal aortic aneurysm surgery, aortic ileus surgery, varicose vein surgery, carotid artery surgery, pulmonary surgery, emergency thoracic surgery, and cardiac surgery.

In 2001, he participated in the establishment of the Cardiovascular Surgery Clinic at SSK Surya Pasha Hospital and contributed to the establishment of the Cardiac Surgery Department. Professor Ali Fedhakar also established cardiovascular surgery clinics at Giresun and Sinop State Hospitals, where he performed the first surgeries.

From 2003 to 2005, he worked as a Competent Surgeon at Isvicere Hospital and from 2005 to 2017 as a Competent Adult Cardiac Surgeon at Koşuyor Heart Hospital. During this time, he performed approximately 4,000 open-heart surgeries. He was a member of the heart transplant team at Kartal Koşuyolu High Specialty Educational and Research Hospital from 2010 to 2015, where he was the chief surgeon, harvesting hearts from donors in about 40 transplant patients and performing heart transplants in 8 patients.
