One possible explanation for spontaneous regression is that the body initiates an immunological response against specific antigens on the surface of tumour cells. On the other hand, tumours are notoriously variable, both in heredity and behaviour, which can result in unrelenting disease progression in some people while causing spontaneous remission in others.

Tumours of the same sort (for example, breast cancer) might mutate in a variety of ways. This can affect the rate of tumour growth, the chance of metastasis to other areas, and the response to treatment.

Another explanation: a rare childhood cancer can throw some light on how genetic changes can cause cancerous regression.

Neuroblastoma is a childhood cancer and has two variants, including Type-1 and Type-2.

As opposed to type 2 tumours, type 1 neuroblastomas have very low levels of telomerase activity. Telomerase regulates the length of specialized sections of DNA that allow the cell to divide indefinitely. Because of the low activity of the enzyme, they are relatively short and unstable in type 1 neuroblastomas, resulting in cell death.

Although the specific mechanisms behind spontaneous regression remain unknown, it is highly likely that in people with certain genetic profiles, triggering a powerful immune response must play a significant role.

According to the Journal of the American Medical Association, screening appears to be detecting a large number of small cancers that would not be a problem if left alone, as well as tumours that would be fatal if left untreated. They were destined to stop growing on their own, shrink, or even disappear, as in the case of some breast tumours.
